Where does it hurt video template

A pain-map intake sheet: a front body outline draws itself, five numbered markers pop onto the sore spots (the worst one pulsing), a legend lists each complaint with a mild / moderate / severe chip, and a 0–10 pain scale fills to 6. Move the markers and rewrite the legend.
